INFO WE NEED:

  1. By law we have to have an image of the Doctor’s original signed Rx to send your Rx order to you.  Make sure the image is whole and nothing is cut off.   Rx MUST NOT BE EXPIRED.

A legal Rx has the doctor’s name, address, patient name, exam date, Rx, and doctor’s signature

  1. PD (distance between your pupils)  REQUIRED                   Get this from whomever made your eyeglasses by phone (good if on Rx itself)
  2. Base curve of your eyeglasses          HIGHLY RECOMMENDED        This should be the base curve of whatever glasses you will be wearing going forward.  Your local optical employees can measure your glasses.  It is simply the measurement of the front curve of your lenses.  If we don’t match your base curve, you may experience an adjustment period every time you use your Rx goggles.  For motocross, BMX, downhill mountain biking and such sports, the base curve is required.  You DO NOT have to see the eye doctor again.  The Opticians that helped you with your eyeglasses can take a lens clock and measure the base (front) curve of your eyeglasses in about 10 seconds.  They will not have a record of it, so you will have to get your eyeglasses to the Opticians.
